The purpose of this case study is to identify barriers to health that impact a patient with diabetes develop a plan of care that includes provider interventions and self-care or self-management strategies for routine management and in times of public health emergencies.
To complete this assignment, review the patient case in the “Diabetes” video and read the National Standards for Self-Management of Diabetes and emergency preparedness readings linked in the topic Resources.
