Which weekly objectives do you have prior knowledge of, and to what extent?
I have prior knowledge of the objectives in Topic 1 including the development of a patient education plan for preventing readmission. Pugh et al. (2021) assert there are strategies such as the 30-day hospital readmissions used to demonstrate the quality of care. Oh et al. (2023) explain discharge education through the teach-back method can be vital in preventing readmissions among patients with heart failure conditions. Rice et al. (2018) explain that nurse-led patient education can be vital for enhancing care for patients with heart failure and minimising hospital readmissions. I also have prior knowledge of the objective in Topic 2 concerning the use of interdisciplinary teams when providing care for a patient with cognitive complexity. Diaz et al. (2023) explain that poor interdisciplinary care team communication can lead to a rise in mortality rates. They identify factors that can improve the functionality of these teams such as role definition, hierarchical effects, informal communication pathways and formal processes. I also have prior experience concerning objectives in Topic 3 about the nurses’ role in supporting the patient’s psychological and emotional needs. Bradshaw et al. (2021) explain the need for warmth and kindness, deep listening and social associations during the treatment process. These are areas in which I have prior knowledge of.
