GETTING STARTED WITH PEDIATRICS

Primary care of children from newborns to those on the cusp of adulthood requires considerable knowledge, as well as the finesse to manage parents and families of all types, and the changing needs, attitudes, and independence of young people as they grow and mature. Considering that this week—in fact, this entire first module—is focused on growth and development, it is fitting for you to reflect on your current and anticipated growth and development in advanced nursing, and specifically, in pediatric care. When you made your decision to pursue training as a Family Nurse Practitioner, how did pediatrics factor in your thinking? Is the opportunity to work with children and adolescents a strong motivator, or does it rank for you as more of a concern?

Strengths and Challenges

I have been a nurse for five years and have worked in a medical-surgical unit and the Operating Room. My strengths include my willingness and determination to learn new concepts, gain hands-on experience performing health assessments, and learn about developmental conditions and diseases seen in the pediatric population. My challenges consist of my limited knowledge of caring for pediatric patients. However, during my first clinical rotation, I was able to care for pediatric patients 21 and younger. In addition, when caring for pediatric patients who cannot voice their concerns, the parents will provide a clue to the symptoms the patient is experiencing. My career goals include graduating with my Family Nurse Practitioner (FNP) degree and passing the boards. My plan to become an FNP was to move away from bedside nursing and the hospital by working in a clinic.

Objectives and Goals

My objectives for this class are to appropriately diagnose pediatric patients based on health assessment and diagnostic tests, formulate a treatment plan, and provide referrals to other specialties. The class focuses on building confidence in assessing, diagnosing, and treating acute and chronic conditions in pediatric populations (Walden University, 2024). This course aligns with my objectives and goals in developing my knowledge of common diseases affecting pediatric patients, diagnosing, and creating a treatment management plan. The following eleven weeks will help to develop my strengths and work on my challenges in caring for the pediatric population. I’ll learn about the well-child visits, health promotion, disease prevention, vaccinations, and congenital conditions.

Roles and Responsibilities of Pediatric Nurse Practitioners

Although there has been a rise in nurse practitioners, there remains a disparity in pediatric nurse practitioners who can provide high-quality care to patients with chronic illnesses and high acuity (Armstrong et al., 2023). As a family nurse practitioner, I will come across pediatric patients, so it is important for me to gain knowledge about diseases afflicting this population and implement care to help manage their conditions. Pediatric Nurse Practitioners have positive outcomes when working with children with complex developmental conditions and initiating treatments, resulting in improved quality of life, continuity in care management, and increased satisfaction in care (Brandon et al., 2023). Nurse Practitioners have roles and responsibilities to adhere to in providing patient-centered care. Nurse practitioners have a scope of practice that includes diagnosis, treatment, diagnostic tests, referrals, and prescribing medications (Inwood et al., 2021). Furthermore, Nurse Practitioners educate parents and patients and work as a collaborative team with other multidisciplinary roles.

ELIZABETH

Introduction

I reside in Lebanon, Pennsylvania, am married, and have two grown children and four grandchildren. Over the past twenty-seven years, I have worked as a registered nurse in the ICU and hospice. I am passionate about my profession and dedicated to helping those in need. I am looking to continue my nursing career and use my knowledge and experience to make a difference in the lives of others.

Strengths

I have extensive experience in critical care and hospice nursing, which will significantly benefit my growth as a Family Nurse Practitioner and my application to pediatrics. My experience as a registered nurse in the ICU and hospice settings provides me with a solid foundation in clinical skills, patient assessment, and critical thinking. These skills are transferable to pediatrics and can help navigate complex medical cases.

As a dedicated professional, my passion lies in assisting others, such as pediatrics, where I will support children and their families through challenging times. Family nurse practitioners understand that family-centered care involves establishing a partnership between the family and the healthcare professional and respecting and valuing the patient’s cultural background and values (Yoo & Cho, 2020). We are pivotal in promoting health, preventing diseases, and managing various health conditions within the family framework (Dellafiore et al., 2022). With my strong empathy and communication skills, I aim to establish trust and foster meaningful connections with patients and their families, ensuring that their unique needs and cultural perspectives are honored throughout their healthcare journey.

Transitioning from adult-focused care to pediatrics may require some adjustment, but I can adapt to new environments and learn quickly. I am willing to embrace new challenges as I expand my scope of practice to include pediatric care.

Challenges

Despite my years of experience in critical care and hospice nursing, I will face challenges related to pediatric patients’ unique needs and considerations since I have limited pediatric experience. I must gain additional knowledge and skills to provide the best possible care for these patients. I will also rely on my preceptor’s expertise to help me navigate this new territory.

Another challenge is building pediatric-specific knowledge. Pediatrics encompasses various medical conditions, developmental stages, and treatment approaches that may differ from those in adult care. I must invest time and effort in familiarizing myself with pediatric-specific protocols, medications, and best practices to provide optimal care for young patients. Typically, children are viewed as a vulnerable population, so their relationship with families is complex and has the potential for high empathetic engagement (Buckley et al., 2021).

Goals and Objectives

My career aspiration as a Family Nurse Practitioner is to specialize in hospice and palliative care while maintaining flexibility in my options. While my primary focus is on adult care, I recognize the importance of having pediatric knowledge. Should my initial goal not materialize, I am open to the possibility of working with pediatric patients. The coursework in this program plays a critical role in achieving these career objectives. By deepening my understanding of pediatric assessment, diagnosis, and management of common childhood illnesses, I am better equipped to deliver evidence-based care to pediatric patients.

Skills such as effective communication, holistic assessment, pain management, and symptom control are essential in pediatric and end-of-life care settings. Pediatric nursing emphasizes family-centered care, recognizing the importance of involving families in decision-making and supporting the child and their loved ones. This holistic approach aligns with the principles of hospice and palliative care, where the focus is not only on the patient but also on addressing the family’s physical, emotional, and spiritual needs.

Pediatric nursing often involves confronting complex ethical dilemmas and providing emotional support to families facing challenging circumstances. These experiences can help me develop the resilience, empathy, and ethical decision-making skills necessary to navigate the complexities of end-of-life care in hospice and palliative settings.

Conclusion

In conclusion, my journey as a Family Nurse Practitioner is driven by a commitment to providing compassionate, evidence-based care to patients and their families. Through continuous learning and engagement in my nurse practitioner program, I am confident in my ability to contribute positively to the health and well-being of all patients. By integrating the knowledge and skills gained from this pediatric course, I can develop a comprehensive skill set to excel as a Family Nurse Practitioner specializing in end-of-life care.
