The observation of professional nurses offering care within the community demonstrates that possible and actual risk factors impact individuals and families. These risk factors are physical, social/cultural, individual behavior, and community organization. For instance, physical factors like built environment, industrialization level, and geographical location affect individuals and families. Individuals and families who live close to industrial areas may have elevated risks for pollution that affects their health (Sherwood, 2021). Social/cultural risks like poverty, mental illness, unsafe housing, malnutrition, and low literacy levels as well as transportation affect individuals and families, thereby increasing their risk factors. Individual factors like suffering from chronic conditions like diabetes, lack of health insurance coverage, and low income leading to not accessing healthcare are some of the possible risk factors that professional nurses will encounter during health assessment.
Interventions by the Professional Nurse
The professional nurse implements evidence-based interventions within the community setting to promote health and prevent diseases. These interventions include health evaluation, provision of health education, development of health materials, and engagement of community stakeholders, families, and individuals to identify risk factors and other aspects that hinder access to health care. Professional nurses also plan patient care, education, and support as well as advocacy activities to improve the overall health and wellness of health populations and individuals (Sherwood, 2021). The use of evidence-based practice (EBP) health interventions and promotion allows nurses to improve care outcomes and accessibility for health populations and individual patients.
Roles and Responsibilities of Professional Nurses in Community Health
Nurses play a critical role in providing care within a community setting. Community nurses offer clinical services to patients away from hospital settings, assisting them with illness, treatment management, and recovery. Community health nurses offer bespoke care to individuals, families, and households in their homes and local areas leading to reduced admissions and pressure on health infrastructure. Therefore, their core role is to provide care and coordination with the healthcare system and reduce the health burden. The core responsibilities include care coordination, planning, advocacy, and implementing interventions to reduce disease occurrence and poor patient outcomes (Ho et al., 2022). As patient advocates, professional nurses within the community setting protect patient’s medical, legal, and human rights by providing sufficient care interventions to improve outcomes and care delivery. Disease surveillance and health promotion are also critical responsibilities for professional nurses within the community setting.
Integration of QSEN Competencies
As nurses provide care within the community setting, they should integrate core competencies based on the Quality and Safety Education for Nurses (QSEN) framework. These competencies include patient-centered care, teamwork and collaboration, evidence-based practice, quality improvement, safety, and informatics (Eder et al., 2021). As observed during the care process, the professional nurse integrated all the competencies. For instance, the nurse worked through teams and collaborated with all stakeholders, used evidence-based practice interventions, and focused on quality, and safety to deliver patient-centered care in all instances of the delivery process. For example, the nurse implored patients without health insurance to enroll in Medicaid to benefit from enhanced access to medications and treatment protocols.

The professional nurse plays a pivotal role in community settings based on their approaches to care demands and needs within health populations and among individuals. As they work based on their roles and responsibilities, professional nurses strive to meet the QSEN competencies. The observation of the professional nurses working within the community demonstrates that they meet all these requirements.
