Select a family, other than your own, and seek permission from the family to conduct an interview. Utilize the interview questions complied in your interview questionnaire to conduct a family-focused functional assessment. Document the responses as you conduct the interview.
Upon completion of the interview, write a 750-1,000-word paper. Analyze your assessment findings. Submit your questionnaire as an appendix with your assignment.
Include the following in your paper:
Describe the family structure. Include individuals and any relevant attributes defining the family composition, race/ethnicity, social class, spirituality, and environment.
Summarize the overall health behaviors of the family. Describe the current health of the family.
Based on your findings, describe at least two of the functional health pattern strengths noted in the findings. Discuss three areas in which health problems or barriers to health were identified.
Describe how family systems theory can be applied to solicit changes in family members that, in turn, initiate positive changes to the overall family functions over time. Part 2 of the assignment is as follows
Refer back to the interview and evaluation you conducted in the Topic 2 Family Health Assessment assignment. Identify the social determinates of health (SDOH) contributing to the family\’s health status. In a 750-1,000 word paper, create a plan of action to incorporate health promotion strategies for this family. Include the following:

Describe the SDOH that affect the family health status. What is the impact of these SDOH on the family? Discuss why these factors are prevalent for this family.
Based on the information gathered through the family health assessment, recommend age-appropriate screenings for each family member. Provide support and rationale for your suggestions.
Choose a health model to assist in creating a plan of action. Describe the model selected. Discuss the reasons why this health model is the best choice for this family. Provide rationale for your reasoning.
Using the model, outline the steps for a family-centered health promotion. Include strategies for communication.
Cite at least three peer-reviewed or scholarly sources to complete this assignment. Sources should be published within the last 5 years and appropriate for the assignment criteria.

Family Focused Assessment
Families play an important role in any society. They contribute to the health and wellbeing of a society. Families consist of individuals that work together towards achieving a common goal. The individual behaviors are important in determining the overall health of the family. Therefore, this essay is a summary of interview that was conducted in a family to determine family behaviors. The essay explores areas that include a description of the family structure, functional health pattern strengths and weaknesses, and effectiveness of family systems theory in promoting positive change in the family.
Description of the Family Structure
The selected family that was used in undertaking this assignment is Mr. X’s family. The family is an extended family. The family members comprised of Mr. X and his wife, two children, and their grandparents. The family is African-American family. The husband, Mr. X works in a local manufacturing company as a salesperson. His wife works as a cashier at a local restaurant. Their children – two daughters- are in their junior high school. The grandmother is a retired nurse while the grandfather is an ex-military person. The family members are Christians who attend the church on a regular basis. The family can be categorized as a middle class family based on their lifestyle and affordability of the basic needs and wants. The home environment of the family was clean. There was adequate supply of clean water, adequate food supplies, and effective disposal of wastes.
Overall Health Behaviors of the Family
An interview to identify the health behaviors of the family was undertaken using Gordon’s functional health patterns. The interview revealed that the family perceived their overall health to be good. The family members reported to understand the importance of health promotion. They utilized the available health care services that include screening and early treatment of health problems. However, the perceived challenges of the family in accessing the care they needed included high cost of medical care, use of traditional medicines, and lack of understanding about some of their health problems. The interview revealed that the nutritional status of the family was adequate. The adequacy was seen from the ready availability of healthy diets for the family members. There was also the evidence of minimal influence of culture on the family’s dietary habits. The family however noted that having elderly members had affected the dietary choices they made since they prioritized the health needs of their grandparents.
The interview also revealed that most of the family members have adequate rest and sleep. This could be seen from the average number of hours that the family slept or rested on a daily basis. The grandparents however reported to experience insomnia. They reported to have challenges in getting and maintaining sleep. The family reported minimal issues related to elimination. The grandparents were reported to have elimination problems that included incontinence and use of laxatives to ease passing of stool by one of the grandparents. This experience has been a source of significant distress to the family. The family also reported to engage in adequate physical activities. They included having a morning run and assisting their grandparents walk round the compound on a regular basis. One of the grandparents however had spinal injury, which limits his ability to engage in moderate physical activity.
The interview also revealed that the family members had a positive perception towards self, others, and their bodies. They appreciated themselves for helping one another achieve their set goals in the family. No abnormalities were identified in the sensory-perception aspect of assessment. The family members were aware of themselves, space, and time. They also played active roles in their desired social, functional, and occupational roles. However, the grandparents were dependent on their children. The family members reported to utilize social support systems in addressing their stressful conditions. They assist each other in seeking ways of managing their stressful events. They also seek the support of the church members and pastor in difficult situations. The interview did not reveal any sexuality-related problems in the family.
Functional Health Pattern Strengths, Barriers or Health Problems
One of the functional health pattern strengths of the family is their enhanced perception towards their health and values. According to Edelman, Mandle and Kudzma (2017), health perception focuses on the understanding of the patient, individuals or families about their health needs and ways of meeting them. Individuals or families that have enhanced health perception have improved health outcomes due to their empowerment to engage in activities that promote their health. For example, the family members engage in health promotion interventions such as screening for diseases and early management to prevent undesired health outcomes. The selected family for the case study understood their role and responsibility in engaging in activities that promote their health.
The second functional health pattern strength for the family is effective coping mechanisms to stressors. The family reported to have effective mechanisms in which they handle their stressors. Accordingly, they utilize their existing social support systems to address their stress. As shown by Doyle, Ward and Early (2018), social support systems provide the vulnerable with the resources that are needed in responding to stress. They assist them in identifying effective ways in which stress can be managed. They also empower them to adopt adaptive strategies to responding to stress. Mr. X’s family reported using support from the family and church members alongside their pastor to address their stressing situations. The utilization of these social support systems promotes the health and wellbeing of the family by minimizing the risks of adverse outcomes such as depression and family conflicts.
Two barriers to health care were identified from the interview that was conducted in Mr. X’s family. One of them is the effect of cost of health care services. The family perceived that their ability to access the care that they need was limited by high costs of care. According to them, some aspects of healthcare are expensive for the family. The barrier that was identified from the interview is lack of understanding of some health problems that affected them. The lack of understanding affected negatively the family’s ability to utilize the existing healthcare services. The other area of health problem that became evident in the interview is the high rate of dependence in the family. The grandparents depend on their family for the care they need. The dependence has a negative effect on the family by affecting its socioeconomic development (Barry et al., 2017). The resources that could have been used for development are diverted to address the health needs of the elderly in the family.
Use of Family Systems Theory to Solicit Changes
The family systems theory can be used to solicit changes in the family members and initiate positive changes in the overall functions of the family. The family systems theory postulates that it is difficult to understand individuals in isolation. Instead, enhanced understanding is only achieved when they are considered as part of a family. The theory also asserts that families exist as interconnected systems. Individuals depend on one another to achieve their diverse goals. This theory can therefore be used to promote positive changes in Mr. X’s family. Mr. X’s family can be made to understand that family balance can only be achieved if its members work in unison towards achieving a common family goal. It can be made to understand the importance of independence in achieving the diverse individual and family goals. Through this theory, the family understands the significance of social support system in addressing the needs of the family (Keller & Noone, 2019). Therefore, an effective use of the family systems theory will promote positive change in the family by strengthening the need for oneness and interdependence among family units.
Overall, the interview that was conducted in this assignment revealed a number of strengths as well as weaknesses in Mr. X’s family. Some of the strengths include the presence of a strong social support system, enhanced perception of health, coping, sexuality, activity exercise, and role function among others. Weaknesses such as the high cost of care and dependency rate in the family were however identified. Therefore, it is important that these weaknesses be addressed with effective use of the family systems theory.
