I will explore different aspects of care coordination and the issues surrounding care coordination. The strategies for effective collaboration with patients and families to achieve desired outcomes will be explored. Various aspects of change management directly affect the elements of patients’ experience, which are essential for providing high-quality patient-centered care. The rationale for coordinated care plans based on ethical decision-making will also be discussed. We will also examine specific healthcare policy provisions and how they affect patient experiences and outcomes. Finally, we will discuss our role as nurses in care coordination and the continuum of care.

Understanding Care Coordination

According to the Agency for Healthcare Research and Quality (AHRQ), care coordination is defined as the deliberate organization of patient care activities and the dissemination of information among all the participants concerned with a patient’s care, thus promoting the achievement of safer and more effective care. Coordination of care puts the patient’s needs and preferences before care delivery and communicates them to other players in the team to enhance effective and safe care. Some of the care coordination approaches we can use to enhance safe and effective care include care management, teamwork, medication management, patient-centered medical home, and health information technology (AHRQ, n.d.). There are also specific care coordination activities, including knowledge sharing/communication, supporting patients’ self-management goals, creating a proactive care plan, and assisting with transitions of care.

Nurses play a significant role in the care coordination process, from employing care coordination approaches to facilitating care coordination activities. Thus, it is essential to understand the concept and know what is expected of us.

Effective Strategies of Collaborating with Patients and Families to Achieve the Desired Health Outcomes

Various effective collaboration strategies can be used by nurses to collaborate with patients and their families, thus enhancing health outcomes. Different strategies work best for different patient needs and situations, hence the importance of assessing a particular patient and their needs and identifying the best collaboration strategies to be used. I will discuss three collaboration strategies in this presentation: collective goal setting, open and free communication, and collective planning and shared decision-making.

Collective goal setting is a collaborative strategy that enhances desired health outcomes since the patient and the care provider agree on what to work on, thus enhancing acceptance and adherence to the plan. According to Holmes et al. (2019), collective goal setting also allows the patient to air their concerns, needs, and preferences, which are factored in in the goal setting, thus enhancing better health outcomes.

Open and free communication is required to facilitate the interaction between the care provider and the patient/ family members. It enhances understanding between the care provider and the patient and clarifies issues. Hart et al. (2020) note that open and free communication helps build trust between the patient, family, and care provider, which promotes adherence and thus improves health outcomes.

Collective planning and shared decision-making is an essential collaboration strategy that ensures the patient and the care provider understand the care plan used in an interaction. Additionally, it helps include the patients’ needs and preferences in care planning, thus promoting patient participation and adherence.

Change Management Aspects Directly Influencing Patient-Centered Care

Various aspects of change management directly affect the elements of patient experience essential to providing high-quality patient-centered care. The healthcare field experiences changes over time, which requires care providers to strategize and plan on managing change and making the impact of the changes long-term. One of the aspects of change management that directly affects patient-centered care is training. All the entities affected by the proposed change should be trained in preparation for the impact of the change and readiness to accept and embrace change.

According to Vaishnavi, Suresh & Dutta (2019), training before implementing change in healthcare institutions helps the care providers embrace change easily and prepare adequately for the impact of the change. It also helps in making the change impact long-lasting. Certain changes in healthcare institutions considerably influence patient experience and patient satisfaction. Patient experience and satisfaction are some of the major aspects of patient-centered care, whose changes directly influence care delivery. Some aspects of patient experience that impact patient satisfaction include communication, easy accessibility of information, and timely appointments (AHRQ, n.d.). Therefore, training nurses on the impact of proposed change initiatives is essential, especially when the initiatives directly affect patient-centered care.

Coordinated Care Plans and Ethical Decision-Making

Coordinated care plans are developed to enhance patient participation in care and the participation of other players in healthcare provision. Coordinated care plans must involve ethical decision-making, which means that the nurse should consider the ethical principles of care provision when developing coordinated care plans. For instance, the coordinated care plan developed should be developed based on decisions that will prevent patient harm, considering the non-maleficence principle. It should also be decisions that benefit the patient, thus considering the principle of beneficence. Our role is to ensure that we maintain ethical conduct in all practice activities. Ensuring ethical decision-making in coordinated care plans is therefore crucial.

An ethical approach to care enables nurses to maintain the required conduct of nursing practice. Additionally, it helps promote human dignity and maintain human rights. An ethical approach to care helps nurses avoid ethical implications that would have adverse consequences in their practice (Jin et al., 2021). The underlying assumption in applying ethical decision-making in coordinated care plans is that all nurses know the ethical principles and conduct they are supposed to maintain.

Potential Impact of Healthcare Policy provisions on Outcomes and Patient Experiences

One healthcare policy provision that may potentially impact outcomes and patient experiences is the Health Insurance Portability and Accountability Act (HIPAA). HIPAA regulates the dissemination of protected health information by mandating the application of privacy and security safeguards. Care coordination involves a lot of information dissemination between the different health players and the patient/family. The act can influence outcomes and patient experiences by enhancing patient safety through privacy and confidentiality.

Nurse’s Role in the Care Coordination Process

Having understood care coordination, the strategies for effective collaboration, and the various implication of policy provisions on care coordination, it is vital to explore our role in the care coordination process. Nurses are primary care coordinators and essential players in the process. Since we spend more time with the patients and are the care providers closest to them, it is our role to actively involve the patient and the family in the care coordination process. Providing information and clarifications through open and free communication will help make the process more effective and facilitate patient-centered care. As patient advocates, our role is to ensure that patients’ needs are factored in in the coordinated care plan, thus enhancing patient outcomes.

Conclusion

In summary, care coordination is essential in enhancing patient-centered care. We have seen the critical role we should play as nurses. Identifying collaborative strategies to facilitate the process and engage patients and families is essential. We should understand the change management aspects that influence patient experiences and satisfaction. Government policies that influence care coordination should be considered in the process. Our role is to ensure that patient needs are factored into the care coordination process.

All nurses must be cognizant of the care coordination process and how safety, ethics, policy, physiological, and cultural needs affect care and patient outcomes. As a nurse, care coordination is something that should always be considered. Nurses must be aware of factors that impact care coordination and of a continuum of care that utilizes community resources effectively and is part of an ethical framework that represents the professionalism of nurses. Understanding policy elements helps nurses coordinate care effectively.

This assessment provides an opportunity for you to educate your peers on the care coordination process. The assessment also requires you to address change management issues. You are encouraged to complete the Managing Change activity.
