Population health is a broad concept encompassing elimination of diseases and injuries and the factors that influence health. It focuses on factors that impact health, like the environment, policy and governance, education, poverty, racism, infrastructure, access to technology, and urban planning. Community health is the art and science of promoting health and preventing disease through community participation. It seeks to: Increase lifespan, decrease morbidity rates, increase the rate of adjustment of an individual to his environment, and provide total health care to enrich quality of life. Public health focuses on improving a population’s health and well-being, similar to population health. However, it does not take a holistic approach and thus does not include determinants of health.
Walton et al. (2022) explain that population health focuses on the interpersonal nature of health outcomes that affect a group of people, including the distribution of those outcomes within the group. A population health approach is essential in promoting health and wellness since it targets the social and economic contributors to poor health outcomes. The basis of population health is that the health outcomes of a group of individuals are influenced by social and structural contributors to health, like structural racism and the environment. Consequently, the population health approach targets these factors to improve health outcomes for the particular group.
The nurse’s role in population health includes helping people identify health problems and goals and develop and implement strategies to reach their goals. The nurse assesses the group’s health status using data, community status identification, and input from community service (Walton et al., 2022). Besides, the nurse collaborates with the group to identify the possible expected outcomes. Lastly, the nurse promotes and supports the development of policies that would provide interventions to improve the health status of specific groups of people.
