Regardless of political affiliation, every citizen has a stake in healthcare policy decisions. Hence, it is little wonder why healthcare items become such high-profile components of presidential agendas. It is also little wonder why they become such hotly debated agenda items.

In Part 1 of this module’s Assignment, you were asked to begin work on an Agenda Comparison Grid to compare the impact of the current/sitting U.S. president and the two previous presidents’ agendas on the healthcare item you selected for study. In this Discussion, you will share your first draft with your colleagues to receive feedback to be applied to your final version.

Mental illness not only affects the person who is mentally ill, but also their families, communities, and the national ecomony.  One study reports that mental illness has a multidimensional impact on families dealing with Severe mental illness (SMI). Some of these effects are physical health problems, stress, socioeconomical issues, and psychological difficulties (Fekadu, W., Mihiretu, A., Craig, T. K. J., & Fekadu, A., 2019). For children who live with persons with SMI, the impacts could be performance issues in school, nutritional problems, and higher mortality rates (Fekadu, W., 2019). Communities are impacted indirectly, usually by cost incurred from loss of productivity and absenteeism in jobs (Agboola, A. A., et al., 2018). The effects on the national economy are from indirect (as above) and direct costs which are things such as treatments transportation, and costs of insurance (Agboola, A. A., et al., 2018). Agboola et al., states “Indirect costs are challenging to quantify but are critical for informing public policy and guiding discussion on what should be invested to prevent or treat the illness” (2018). This statement is what drives the Presidential agenda and budget for illness such as mental health.

Your grid was put together well and easy to understand. I can appreciate your topic as it relates so closely to the topic I chose of the Opioid epidemic. These two health issues cost tax payers billions of dollars every year. I for one am interested in results or data. The Presidential budget allots monies for both health issues yearly, trending upward every year. But, are we actually seeing good outcomes? Statistics on opioid overdoes continue to increase every year, yet we are dumping $34 Billion dollars into efforts to stop the problem. I have no issue supplying the money, however I would like to see better outcomes from our dollars spent. So for your topic, what are the outcomes? Are the issues trending upward or are you seeing improvement with the allocated dollars?

Hello K. I found your comparison grid interesting as I to have chosen to address the mental illness topic.

You mentioned that President Trump was very supportive of the research and implementation of mental health services.  According to the American Public Health Association, President Trump released his fiscal year 2020 federal budget proposal in March, recommending huge cuts across the federal government, which includes a budget proposal of a $777 billion cut to Medicaid over 10 years, as well as nearly $600 billion in net Medicare spending reductions, as well as cutting low-income housing assistance, and 1 billion cut to the Health Resources and Service Administration, and a $65 million cut in the Substance Abuse and Mental Health Services Administration (Kim Krisberg, 2019).

I find this proposal to be a threat to the already steps that have been accomplished by President Bush, and President Obama. Medicare and Medicaid already are currently the largest payers of behavioral health services in the country (Caren Howard, 2019). President Trump’s 2020 fiscal proposal for cutting Medicaid and Medicare services will greatly affect those with mental disorders and substance abuse issues from getting the help and services that they require.  For me, this is like taking a step backward.

Part 1: Agenda Comparison Grid

Mental Illness is selected as the Population Health concern.
Describe the Population Health concern you selected and the factors that contribute to it.
Mental Illness refers to various types of mental problems inhibiting the functionality of the mind, such as thinking, behavior, mood, emotions, and so forth mental patterns. Anxiety, depression, social injustice, drug abuse, and other factors contribute in increasing the susceptibility and vulnerability among the individuals to experience mental illness (Fuhrer & Keyes, 2019).

Describe the administrative agenda focus related to this issue for the current and two previous presidents.
Although Donald Trump focused on improving health in all domains in the campaign, he later focused the administrative agenda more strictly towards eliminating the opioid epidemic after El Paso and Dayton. He further asserted in his agenda to talk more about mental health – marking it the need of the hour. However, apart from the discussion, the focus on passing beneficial bills and expanding parity had been limited in his era (Achenbach et al., 2020).
Barack Obama extended the vision of Bush in providing more easy and affordable mental health access to the individuals with mental disorders. He not only extending the Medicaid service to mentally challenged people, rather increased the federal support in housing. Furthermore, he set in his agenda to conduct dialogues on mental health at state and federal level. Moreover, he extended federal support in various domains, such as housing, etc., to the mentally ill people (Hart, 2016).
George W. Bush for the first time, included mental health on a priority list, and formed a New Freedom Commission on Mental Health to facilitate a debate, in which experts provided 19 recommendations. Furthermore, he included in his administrative agenda to equalize the importance of mental health to that of physical health. Besides, his administrative agenda included treatment and crime prevention corrective measures among the mentally ill individuals (Hart, 2016).

Budget cut in the funding for health, especially eliminating the longstanding policy of compensation by federal to Medicaid eligible individuals receiving mental care through Medicaid programs.

Furthermore, he limited $5 billion budget in federal funding to states for this imperative Population Health issue (Achenbach et al., 2020).

i. Patients Protection and Affordable Care Act (ACA): Obama signed this bill in 2010, which aimed at extending the federal mental health parity to the existing Medicaid programs, hence facilitating the mentally ill patients to receive affordable and convenient care.

ii. Frank Mehlville Supporting Housing Investment Act: This Act that was signed in 2011 aimed at increasing the federal support for mentally challenged individuals to enable them in buying house with federal support.

iii. National Conference: A dialogue on Mental Health and national level was held in the White House (Hart, 2016).

With Mentally Ill Offender Treatment and Crime Reduction Act (MIOTCRA) and Paull Wellstone and Pete Domenici Mental Health Parity and Addictions Equity Act during the era 2004 to 2008 marked physical illness equal to mental health practically for the first time, as the federal coverage was equally allocated without discrimination. Furthermore, the corrective measures were planned based on the New Freedom Commission on Mental Health, whereby, experts provided 19 recommendation to improve mental health situation in the US (Hart, 2016).

No extra finance other than the budget and federal coverage. Not much focus on providing resources, such as establishing institutions and funding concerned researches (Hart, 2016).
Explain how each of the presidential administrations approached the issue.
There is a lack of focus by administration in practically improving the mental health situation, which is one of the biggest Population Health concern. According to Achenbach, McGinley, Goldstein, and Guarino, “The administration so far has been more talk than substance on the issue.” (Achenbach et al., 2020, par. 13)
The focus was clearer in eliminating the discrimination between physical health and mental health. The approach was more focused at improving the overall mental health situation of those vulnerable or affected by extending the Medicaid program and federal funding. Besides, to curb the social injustice – one of the prominent factors contributing in mental disparity – the federal support was provided to mentally challenged individuals to fulfill their acquisition of buying a house. For the first time, measure was taken to improve the situation by eliminating the root causes (Hart, 2016).
The approach was significantly better although not the best, provided the growing issues pertaining to health problems, such as growing suicide rate among soldiers who were sent to Afghan War, and anxiety and fear due to 9/11 attack. However, a practical measure was taken, which showcase a rather constructive approach, especially when federal coverage was equally provided to mental illness compared to physical illness (Hart, 2016).
