Pathophysiology  Surges of estrogen and progesterone result in vasodilation. Plasma blood volume expands, resulting in increased heart rate. Elevated heart rate and stroke volume increase cardiac output (Wilkerson,2019). -This condition results from sustained elevated blood pressure within the veins, resulting in leakage of blood contents into adjacent tissue. In lymphatic failure, this mechanism is compromised, resulting in fluid accumulation within the lymphatic system. It occurs due to the destruction of liver architecture by fibrous bands around regenerative nodules, resulting in vascular injury and biliary obstruction and causing hepatocyte injury and death.

 

Primary medical concerns  Risk for development of eclampsia, HELLP syndrome, or chronic hypertension. Patient risks complications such as cellulitis, superior vena cava syndrome, tracheal and bronchial obstruction. Patients develop complications over time, such as portal hypertension, liver failure, and hepatocellular carcinoma (Ye,2022).
Primary psychosocial concerns Patients show higher levels of depression and anxiety -physical effects may interfere with one’s lifestyle and work

-Body image and esteem issues may develop

-Patients may develop stress, depression, and anxiety due to fear of the progression of the disease and increased dependence on their caregivers

 

Implications for critical care and advanced practice nurses -Management helps prevent progression to severe hypertension and maternal complications such as heart failure. -Detailed physical history and clinical exams need to be conducted to determine the cause of the pathology and rule out malignancies or undetected allergies and infectious processes. -Healthcare providers  give support and guidance for the management of alcohol abuse

-Assessment of other associated comorbidities such as diabetes and obesity.
